Accessibility and learning support enquiry

To receive accessibility and learning support you will need to register with our service.

We recommend that you register as soon as you have enrolled, as some of the assistance we can provide requires early planning.

You can start the registration process by completing the online accessibility and learning support enquiry form here and uploading verification of your disability or ongoing medical condition.

The information you provide on this form is confidential and will be kept within Accessibility and Learning Support.  It will assist us to provide a prompt and personalised response to help ensure the right support is in place for your studies.

Information about the nature of your disability or medical condition will not be disclosed and will not appear on your academic records.

Medical Documentation
Do you have recent medical documentation that outlines your diagnosis and impact on your ability to study?

If you do not have medical documentation outlining your diagnosis and/or imapct on your ability to study, you will need to complete the Health Practioners Report (209 KB) (PDF document) and bring the completed form to your meeting with the Disability Support Coordinator. This completed form is required before your Learning Support Plan can be finalised.
